Job Title: Community Health Worker

We are seeking a passionate and dedicated Community Health Worker to join our team. As a Community Health Worker, you will be responsible for for helping patients and their families to navigate and access community services, basic resources, and adopt healthy behaviors. The CHW supports Providers and the Case Managers through an integrated approach to care management and community outreach.

Responsibilities:

• Build rapport and connection with patients in an empathetic manner
• The CHW will screen patients for SDOH issues using the PRAPARE questionnaire and make service referrals or enroll patients in services as appropriate. The CHW will also assist with supportive services as needed such as transportation, housing, etc.
• The CHW will assist patients in scheduling, accessing, and utilizing health services with their primary care provider (PCP). If a patient does not have a PCP, the CHC will work to get the client established with a PCP
• The CHC will attend community groups, participate in health fairs and distribute educational and health materials where appropriate
• The CHW services will include; screening, counseling, eligibility assistance, referral, follow-up, and other services designed to assist RHC target patients in establishing eligibility for and gaining access to Federal, State, and local programs that provide or financially support the provision of medical, social, transportation, housing, educational, translation, and other related services
• The CHC will follow-up to reinforce need to develop the health care provider/patient relationship
• The CHC will Maintain a high level of confidentiality while interacting with patients and clients with empathy and respect
• The CHC will advocate for needs of the targeted community and neighborhood
• The CHC will uphold RHC’s Mission, Vision, and Values
• The CHC will treat community members, patients and other staff members with respect and dignity
• Other duties as assigned

Requirements:

• High school education or GED requested. Experience and skills may substitute for formal education
• Knowledge of the targeted community, available resources and barriers
• Able to work with people of diverse socio-economic and cultural backgrounds
• Effective oral and written communication skills
• Skilled in group process, problem solving, decision making and case management
• Respect for cultural and spiritual diversity
• Transportation with valid Indiana driver’s license and proof of automobile insurance
• Personal knowledge of the community and its members having either lived in or around the area (currently or in the recent past)
• Friendly, outgoing personality
• Bilingual Preferred but not required
